Set Up A DMM Bulk Fuel Account

1. A service charge of 2.0% per month (26.8% per annum) calculated and payable monthly will be applied on any amount which is past due.

2. The company account is considered delinquent if not paid in full by the due date. Additional deliveries may be suspended until past-due amounts are paid in full (unless other arrangements are approved). Any invoices past due are subject to listing for collection with the Credit Bureau.

Pre-authorized payment: I/we enclose a sample “VOID” cheque for the account to be used.

This PAD may be cancelled at any time by sending a request in writing via email to ar@dmmenergy.ca or via Canada Post to DMM Energy, 129 Wheatfield Road, Centreport, MB R4B 0A3. A minimum of 2 weeks’ notice is required.

You may contact DMM Energy at the above e-mail address or by phone at 204-694-8007 to make inquiries, obtain information, or seek recourse in the event of an error or improperly authorized PAD.

You have certain recourse rights if any debit does not comply with this agreement. For example, you have the right to receive reimbursement for any PAD that is not authorized or is not consistent with the terms of this PAD Agreement. To obtain more information on your recourse rights, you may contact your financial institution or visit cdnpay.ca.

DMM Energy will contact you for your Credit Card details once your account has been successfully set up.

• The purchases will be charged to the credit card above, and the charged will be processed immediately for each transaction without signature(s);
• This authorization will remain in effect until I give at least seven (7) days written notice of its cancelation, or any change in the above information to DMM Energy and DMM Energy has acknowledged receipt;
• DMM Energy may process a charge for the total amount outstanding, if any, prior to the effective date of such cancellation;
• DMM Energy reserves the right to, at any time;
  • (a) refuse any such purchases by the foregoing person(s) and/or
  • (b) refuse or discontinue this payment option
• DMM Energy will not accept credit card payments on overdue or outstanding accounts.
